Try using mii-diag to force the PHY into particular states.
http://cesdis.gsfc.nasa.gov/linux/diag/index.html has the source and a
brief how-to.  Looking at the source (0.91m for me), I don't see the
PHY layer being init'd from (re)starting the interface, that all seems
to be done in tulip_probe1() at the time of the module load.

If nothing else, you can use mii-diag to see what the registers are
before and after the fault, and perhaps get a clue.
