Problems with D-Link DFE-660tx PCMCIA Ethernet card

I have the following problem with a D-Link DFE-660tx PCMCIA Ethernet
card.
When I started to use the card it worked and the card was recognized as

The card worked and I was happy.
But later the problems started and the card was sometimes not recognized
correctly. I upgraded from pcmcia 3.0.14 to various versions of 3.1 but
the card worked only sometimes.
Now the card is always recognized as

eth0: Digital DS21143 Tulip rev 73 at 0x400, 00:80:00:80:00:80, IRQ 9.
eth0: EEPROM default media type 10baseT.
eth0: MII transceiver #0 config 3000 status 7809 advertising 01e1.
The chip revision seems to have changed and the MAC address is wrong.
Does anybody know what is going on?
I see in tulip_cb.c that revision 65 is handeled differently. Is it
possible
to skip the call determining the revision and set it to 65? Or could
this
be of harm for the card.
Is it possible that the driver may harm the network card?
