Oops, the underlined "I" in the link looked like a "J" to me. I'm sorry...

As far as the card with the device ID of 0x0008, I exchanged it for an identical card (same shelf, even.) The new card reports a device ID of 0x0009 like it should and works fine. Perhaps I just had a flaky card.
