[tulip-bug] DEC 21040 Stops Working

I have a DEC Multia, which is a tiny alpha computer with a built in DEC
21040 ethernet card.  I am currently running a 2.2.15 kernel.  Every so
often, the ethernet card stops working.  I can't ping it, and it can't
ping other computers.  The only way to get it working again is to remove
the tulip module and then insert it again.

I have also experienced this problem with a 2.2.14 kernel and it has
been happening for quite a while.

What should I look for next time this happens?
